Claims are reviewed in 2-3 business days after they’re submitted. You check the status of your claims on the "Claims" page in your Forma account. (Select “Claims” from the top navigation bar.)
Claim statuses - single claim
Pending
The claim is being reviewed. Claims are usually pending for 2-3 business days after being submitted.
Approved
The claim has been approved and will be reimbursed based on your employer’s policy. If your employer allows reimbursements via direct deposit, you’ll be reimbursed to your linked bank account 5-7 business days after the claim is approved. If your employer reimburses you via payroll, check your employer’s benefit policy for more details on the reimbursement schedule.
You’ll receive an email notification when a claim is approved.
Rejected
The claim was rejected, either because it wasn’t eligible or there was an error with the claim details or the attached receipt.
You’ll receive an email notification when a claim is rejected with more info from our Forma Support team explaining why.
Claim statuses - reimbursement plans
A reimbursement plan is when you submit a claim for an amount greater than the available balance of your benefit. If the claim is approved, you’ll be reimbursed in installments until you receive the entire amount. Please check your company's policy to see if reimbursement plans are enabled.
You’ll first be reimbursed the remaining balance in your benefit, and then as you keep receiving benefit deposits from your employer you’ll be reimbursed those amounts until the claim is completely reimbursed.
In progress
A claim that requires a reimbursement plan has been approved and the reimbursement is in progress.
Completed
A claim that requires a reimbursement plan has been approved and the claim has been reimbursed in full.
Important Note: 'Approved' and 'Completed' statuses both mean that a claim has been approved and no further action is required.
