How do I fix this to unlock my Forma card?
To unlock your Forma card after it has been locked due to an ineligible transaction, link your personal card so the ineligible charge can be processed. Once your personal card is charged, your Forma card will be automatically unlocked.
If the card was locked because a receipt was never submitted, you can submit the receipt with all necessary information so the card is unlocked. You can do this through the transaction via the "pending tasks" notification or via the email sent by Forma.
How does Forma determine if a transaction is ineligible?
Forma’s claims reviewers assess transactions based on your employer’s program policy. If an item or service is not eligible under the policy, the transaction is marked as ineligible.
If a transaction is deemed ineligible due to a missing receipt information, you can contact the merchant for a detailed receipt. Typically, this receipt should include the purchase date, amount, item or service name, and merchant name. Requirements may vary, so check your program policy for your company-specific details.
A transaction can also be considered ineligible if a receipt is never uploaded. The card will be locked 90 days after the transaction if no receipt is provided. Our system sends email reminders to submit the receipt.
