When shopping at places like Amazon, Target, or Walmart, your Forma card might be declined due to the wide range of items these big box retailers sell.
This happens because the Forma card can only be used for items or services that are covered by your employer’s program policy. Since these retailers offer both eligible and ineligible items, our system may not be able to distinguish between them, leading to a decline.
Depending on your program’s guidelines, you may need to upload a receipt to verify if the purchased item or service is eligible. Check your program policy to see if a receipt is required and what information should be included.
Another reason for a decline could be a mismatch between the merchant name and category and our internal system. Our system processes transactions, but if the merchant name or code from the transaction doesn’t match our approval rules, the card may be declined.

- If your Forma card was declined and you need more information, you can contact Forma support with the transaction date, amount, and merchant name for verification.
- If you believe an item or service is eligible, you can pay out of pocket and submit a reimbursement claim with the receipt.
