What is a superbill?
A superbill is an itemized receipt, issued after a service is paid for, of all services provided to you.
Can I submit a superbill claim for reimbursement?
Yes! You can submit multiple receipts under one claim when requesting a reimbursement under your Flexible Spending Account (FSA), Limited-purpose Flexible Spending Account (LPFSA) or Dependent-care Flexible Spending Account (DCFSA). However, the receipts must be within the same calendar month such as all from January or all from February.
Forma does not review superbill claims based on a 30-day period. For example, if you submit a claim from January 16 through February 15, that claim will be rejected and you will be asked to resubmit two new claims to cover January and February expenses separately.
