Claim appeals are eligible under Health Reimbursement Accounts (HRA) and Flexible Spending Accounts (FSA) only.
If you disagree with a claim denial under your HRA or FSA, you can appeal the decision by emailing Forma at appeals@joinforma.com.
You must appeal the decision within 180 days of receipt of this claim denial. Please provide the following information as part of your appeal sent to appeals@joinforma.com:
- claim number
- a brief description of the denied claim
- the reasons that you feel your claim should not have been denied; and
- any additional facts and/or documents that you feel support your claim
Forma will make a determination on the appeal within 15 business days of receipt. We will send a notification of the result to the email address used to submit the appeal.
Appeals for claims under any other account are not applicable and will be denied. For a review of denied claims for non HRA and FSA accounts, information can be sent to support@joinforma.com for investigation.
