Why would I lock my Forma Card?

If your card has been lost or misplaced, you can easily lock it from your Forma account. Once locked, no transactions can be made until the card is unlocked. 

How do I lock my Pre-tax Forma Card?

1. Log in to your Forma account
2. Navigate to your profile in the top right corner and select “View or edit profile”
3. Select “Pre-tax Forma Card” from the menu on the left
4. Select the lock icon

On mobile

1. Log into your Forma account on the mobile app
2. Select "Settings" in the lower right corner
3. Select "Pre-tax Forma Card"
4. Select "My card info"
5. Slide to lock card

How do I lock my regular Forma Card?

On the Web

1. Log in to your Forma account
2. Navigate to your profile in the top right corner and select “View or edit profile”
3. Select “Forma Card” from the menu on the left
4. Select the lock icon (virtual and physical card)

2. Select "Settings" in the lower right corner
3. Select "Forma Card"
4. Click "Lock Card" - the physical and virtual cards are locked separately
