Managing your employer-sponsored post-tax account doesn’t have to be complicated! Here are some quick and easy tips to help ensure everything runs smoothly throughout the year.
Eligibility: What You Need to Know
Before using your benefits, check your company’s Program Policy for details on eligibility. Eligibility questions are one of the most common topics we see!
🔹 Where to find your Program Policy:
- On your Forma homepage
- From your Forma profile drop down menu
- By clicking "What’s eligible?" in any of your benefits > click “Learn more” for additional details
Submitting Claims: Key Guidelines
📝 Claim Submission Deadlines
- Claims must be submitted within the eligible period
- Always check your Program Policy for specific deadlines.
💳 How to Submit a Claim
- File a claim through your Forma account from the web or via the app.
- Each account may have different receipt requirements, so be sure to review your Program Policy first.
- Receipts must include required details—check to see if your name is needed.
- If submitting a dependent expense, add a note in the description to avoid rejection.
- Keep your receipts! Without one, you won’t be able to submit for reimbursement.

Reimbursement Plans: How They Work
⚠️ Not all employers offer reimbursement plans—check your company’s policy.
✔️ How It Works
- Approved claims are reimbursed from your available balance first.
- If the full amount isn’t available, you’ll continue to be reimbursed as funds are deposited into your account.
📅 Timing Example
If your claim is approved on October 12, you’ll receive payments on November 12, December 12, and so on—until the full amount is paid.
🚨 Reimbursement plans may reset at the end of the year or when the full amount is paid, review your program policy for more details.
Using the Forma Card
⚠️ Not all members have access to the Forma Card—check your Program Policy.
💳 Forma Card Best Practices
- Avoid last-minute purchases! Refunds and order changes may not be processed before expiration.
- Try to use your card by the 20th of the month to prevent losing funds due to slow refunds.
- Online shopping tip: If your card is charged when an item ships (not when ordered), it may affect fund availability.
💡 Example: If you order on May 25, but the item ships on June 2, the charge applies to June funds—not May.
Forma Store: Things to Keep in Mind
