Your dependent can use your login if you provide them the credentials (username and password), but they will not have their own login or account.
You have the option to authorize your dependents to access your benefit account information and communicate with Forma on your behalf. To enable this feature, follow the steps outlined below:
- Click on your profile on the upper right-hand side > select 'View or edit profile'
- Click 'Dependents' from the menu on the left
- Click 'edit' next to the dependent you would like to grant this access to
- Scroll down and check the 'permission to speak on the phone' box
If you do not have a password because you usually log in through Single Sign-on (SSO), reach out to support@joinforma.com to request a Magic Link be sent your dependent's email address.
