Used or second-hand items may be eligible depending on your company’s program policy. You can check your employer’s policy on the dropdown menu in the upper right hand corner of your Forma account. The program policy will detail if second-hand items are eligible, receipt details, and type of receipt required.
Generally, when submitting a claim for a second-hand item, include a detailed receipt including the following information:
- Type of product purchased
- Purchase date
- Name of purchaser
- Name of seller
- Price of the product
