If you have a large, pre-paid eligible expense, you may use the reimbursement plan feature to simplify the process. Typical expenses that may qualify include:

Setting up a Reimbursement Plan is as easy as filing a one-time claim. You'll fill out the claim information here. When entering the amount, you'll enter the total amount of the item or pre-paid membership and you'll see the reimbursement plan notification pop up automatically, like the one below.
For example, let's say you have $100 available in your account and you purchase a treadmill that costs $800. Your reimbursement plan request is submitted and approved on January 14th for $100. Every month following, your account will be automatically deducted on the 14th of the month until your claim has been paid in full.
And... that's it! You will be reimbursed in accordance with your company's reimbursement schedule.

**If you have a subscription through Forma and a reimbursement plan at the same time, your subscription may be billed to your personal payment method if a reimbursement plan is scheduled to occur prior to your subscription renewal.
