A subscription is a product or service that requires a monthly fee to access. Common providers in the Forma Store that use a subscription model are:
- Classpass
- Massage Envy
- 24 Hour Fitness
- Crunch Fitness
- Fitness SF
Billing
When you purchase a subscription, Forma will apply your available account balance first then charge any additional amount to the backup payment method you have linked to Forma.
Subscription billing information can be found on the vendor page in the Forma Store and on the checkout page. The billing cycle information for each subscription is listed in the FAQs section at checkout.
Note: Billing cycles vary from offer to offer. Read the details carefully before purchasing a subscription.
Changes and Cancellations
To cancel your subscription, follow these steps:
- From the web: Select the profile icon in the upper right corner of your Forma account and click “Order history” ; From the app: Select "Settings" in the lower right corner
- Select “Subscriptions”
- Select the subscription you wish to cancel
- Click “Cancel Membership”
- You will receive a cancellation confirmation email with the last date of your subscription
