Your Pre-Tax Forma Card can be used to make eligible pre-tax purchases both online and in stores. If a purchase is not eligible, the transaction will be declined. For more details, see Why was my Forma pre-tax card declined?
How In-Store Purchases Work
When you swipe your card at a store, the system reads information from the merchant’s point-of-sale system. The Pre-Tax Forma Card is powered by an Inventory Information Approval System (IIAS), which verifies eligible purchases based on the merchant's internal systems.
For the best experience, we recommend using your physical Pre-Tax Card at the pharmacy checkout with a cashier. This method is generally the most reliable, as it ensures access to the most up-to-date IIAS technology.
