The Pre-Tax Forma Card is a chip-enabled Visa debit card that can be used for eligible HSA, FSA, LPFSA, DCFSA, and Commuter expenses. You can use it anywhere Visa is accepted.

How to Activate Your Pre-Tax Forma Card

Once you receive your card, follow these steps to activate it:

1. Log into your Forma account on the web.
2. Click on your profile in the top right corner and select “Manage Forma Card.”
3. Click “Activate Card.”
4. Click “Request Code.”
5. A verification code will be sent to your registered phone number. Enter the code in the “Verification Code” field and click “Verify.” Note: The verification code is valid for 10 minutes.                                                                                                                                   →
6. Enter the expiration date and security code (CVV) exactly as they appear on your card, then click “Activate Card.”
7. Set your preferred 4-digit PIN and click “Create PIN.”

Your card is now activated and ready to use!
