Sometimes Forma will need to issue you a new pre-tax card. Below are some common reasons this might happen:
- Your active pre-tax card has been used within the last six months and is set to expire next month.
- You have an HSA and enroll in a new non-HSA pre-tax benefit, such as:
- Electing a Limited Purpose FSA (LPFSA) at the start of the new plan year or following a qualifying life event.
- Electing a Dependent Care FSA (DCFSA) at the start of the new plan year or following a qualifying life event.
- Enrolling in Commuter (Parking or Transit) benefits.
- You requested a new pre-tax card through your Forma account.
- If your company transitions away from using Forma as its pre-tax provider, you’ll be able to request a new HSA card through your Forma account once the offboarding process is complete.
- Forma needs to reissue your pre-tax card due to banking regulations.
