How do I update my pre-tax Forma Card PIN?

You can update your Pre-Tax Forma Card PIN through your Forma account on the web. Follow these steps:

1. Log in to your Forma account.
2. Click on your profile in the top right corner and select “Manage Forma Card.”
3. Click “Update PIN.”           
4. Click “Request Code.”
5. A verification code will be sent to your registered phone number. Enter the code in the “Verification Code” field and click “Verify.” Note: the verification code expires after 10 minutes.                                                                                      →
6. Enter your old PIN.
7. Set a new PIN.
8. Click “Reset PIN.”

If successful, you will receive a confirmation pop-up.
