What should I do if I missed the deadline to migrate my HSA to Thread Bank but still want to open an HSA with Forma?

This change only impacts select Forma members. All members impacted will be notified directly via email about account changes. If you have not been notified, then this change does not impact your account.

If you did not sign the agreement to migrate your HSA to Thread Bank before the December 15, 2024 deadline, then you can follow a few steps. Please note there are different instructions depending upon your situation:

  1. If you are NOT enrolled in an HSA for 2025
  2. If you are enrolled in an HSA for 2025

If you want to transfer your HSA funds to Thread Bank but are NOT enrolled in an HSA for 2025, follow the below steps.

  1. You will receive a check from Blue Ridge Bank with the remaining balance in your HSA.
  2. After receiving this check, you should reach out to the Forma Support team at support@joinforma.com with a request to open an HSA with Thread Bank.
  3. The Forma team will ask you to endorse the Blue Ridge Bank check with the following three details (example below):
    • Write “Pay to the order of Thread bank FBO with Forma”
    • A handwritten or stamped signature
    • Date (mm/dd/yyyy)

  1. You will then need to mail the check, along with the HSA rollover contribution form, to Forma’s company address: 47000 Warm Springs Blvd, Ste 1-170, Fremont, CA 94539.
  2. Within four weeks of Forma receiving the check, we will then create the HSA for you and add the funds from the check to the new HSA.

If you want to transfer your HSA funds to Thread Bank and are enrolled in an HSA for 2025, follow the below steps.

  1. You will receive a check from Blue Ridge Bank with the remaining balance in your HSA.
  2. If you are included in the 2025 Open Enrollment file sent by your employer, Forma will open an HSA with Thread Bank.
  3. After receiving this check, you should reach out to the Forma Support team at support@joinforma.com The Forma team will ask you to endorse the Blue Ridge Bank check with the following three details (example below):
    • Write “Pay to the order of Thread bank FBO with Forma”
    • A handwritten or stamped signature
    • Date (mm/dd/yyyy)
