If you didn't previously set up a personal email address or did not receive the Magic Link to your inbox, email Forma Support at support@joinforma.com. Provide your first name, last name, and previous work email address and you will be granted access to your Forma account.
Please note that a $25 account closure fee will apply, or the remaining account balance if the balance is less than $25.
