HSA statements are provided free of charge each month. You can view your past statements from your Forma account any time.
Electronic statements
- Log in to your Forma account
- Go to Benefits and select your HSA
- Open the “Manage HSA” dropdown and select “View Documents”
- Select the download icon to view and download a given statement
