How can I view my HSA account statement?

HSA statements are available free of charge each month. You can access your past statements anytime from your Forma account by following these steps:

1. Log in to your Forma account
2. Go to Benefits and select your HSA
3. Open the “Manage HSA” dropdown and select “View Documents”
4. Click the download icon next to the date to view and download a given statement
