Following termination of employment, you may have pre-tax expenses that you’d like to be reimbursed for. To file reimbursement claims post-termination, go to and enter your personal email address to receive a Magic Link to log into your Forma account.
If you didn't set up a personal email address previously on Forma or you did not receive the Magic Link, reach out to Forma support at firstname.lastname@example.org and provide your first name, last name, and previous work email address. Someone from the Support team will provide you with temporary access to your Forma account.
Claim submission is the only accepted reimbursement method during the post-termination run-out period. Your existing Forma Card cannot be used after termination of employment.
