Following termination of employment, you may have pre-tax expenses that you’d like to be reimbursed for. To file reimbursement claims post-termination, go to https://client.joinforma.com/login?type=magic and enter your personal email address to receive a Magic Link to log into your Forma account.
If you didn't set up a personal email address previously on Forma or you did not receive the Magic Link, reach out to Forma support at support@joinforma.com and provide your first name, last name, and previous work email address. Someone from the Support team will provide you with temporary access to your Forma account.
Understanding Post-Termination Coverage and Run-Out Periods
If your company offers end-of-month coverage, you will be granted a post-termination grace period, allowing you to continue using your Forma Pre-tax Card and incur new eligible expenses through the end of your termination month.
If applicable, a post-termination run-out period will begin after the end-of-month grace period. If end-of-month coverage is not offered, the run-out period begins on your termination date. During the run-out period, claim submission for previously incurred expenses is the only available reimbursement method, and your Forma Pre-tax Card can no longer be used once your employment ends.
If end-of-month coverage and a run-out period are not offered, your termination date is the final day you can use your Forma Pre-tax Card or submit claims.
