This change only impacts select Forma members. All members impacted will be notified directly via email about account changes. If you have not been notified, then this change does not impact your account.
Yes - if you are on leave during the migration, you will still be able to migrate but there are a few details to keep in mind.
Since you are the owner of your HSA (while other pre-tax benefits (e.g. FSA, Commuter) are owned by your employer), we need you as the HSA member to accept updated terms and will make every effort to reach you. Forma will communicate with you via your work email to explain the migration and share action items.
However, if you are unavailable through your work email (e.g., on leave) during the migration, you can reach out to Forma Support at support@joinforma.com and provide your first name, last name, and previous work email address. The Forma Support team can then provide you with a one-time link to access to their Forma account.
