When Forma receives enrollment information for your non-HSA pre-tax benefits (FSA, DCFSA, LPFSA, Commuter)you may receive an email asking to confirm your mailing address, legal name, and phone number. Once confirmed your pre-tax Forma card will be issued. Alternatively, wait until the plan year start and order your card on Forma.
For HSA only members, please request your pre-tax Forma card on Forma on or after the plan start date. You will receive an email to remind you to request a card.
To order a pre-tax Forma card:
- Log in to Forma
- View or edit profile
- Pre-tax Forma Card
- Order new card
There are times when the Forma card needs to be re-issued due to banking regulations.
Your Pre-tax Forma Card will be reissued if you have an HSA but enroll in a new non-HSA pre-tax benefit. For example:
- You only have an HSA and elect a Limited Purpose FSA (LPFSA) at the start of a new plan year or following a qualifying life event.
- You only have an HSA and elect a Dependent Care FSA (DCFSA) at the start of a new plan year or following a qualifying life event.
- You only have an HSA and enroll in Commuter (Parking or Transit) benefits.
