Why lock my Forma card?
If your card has been lost or misplaced, you can easily lock it from your Forma account. Once locked, no transactions can be made until the card is unlocked.
How do I lock my Pre-tax Forma Card?
- Log in to your Forma account
- Navigate to your profile in the top right corner and select “View or edit profile”
- Select “Pre-tax Forma Card” from the menu on the left
- Select the lock icon
- When card is unlocked
- When card is locked
How do I lock my regular Forma Card?
- Log in to your Forma account
- Navigate to your profile in the top right corner and select “View or edit profile”
- Select “Forma Card” from the menu on the left
- Select the lock icon (virtual and physical card)
- When card is unlocked
- When card is locked
