To update your Pre-tax Forma Card PIN, follow these steps:

1. Call 1-833-333-0417
2. Press 1
3. Enter full card number > press #
4. Enter card expiration date: mm/yy > press #
5. Press 1 to change PIN
6. Enter CVV (3-digit code on the back of your card) > press #
7. Enter your date of birth: dd/mm/yyyy > press #
8. Enter your 5-digit billing Zip code > press #
9. Enter new PIN (4-digits) > press #
10. Re-enter new PIN (4-digits) > press #
11. You will hear a message that says “Your PIN has been changed”
