The Pre-tax Forma Card is a chip-enabled, Visa debit card that can be used to pay for eligible HSA, FSA, LPFSA, DCFSA, and Commuter expenses. You can use your Pre-tax Forma Card anywhere Visa cards are accepted.
Activate your card
Once received, call 1-833-333-0417 to activate your card and set a PIN. After initial activation you can change your PIN anytime directly from your Forma account.
