Pre-tax Forma Card
The Pre-tax Forma Card is a prepaid debit card linked to your eligible Forma pretax account(s). You may use the card anywhere VISA is accepted to purchase eligible items and services.
Request a Card
You will get a physical Pre-tax Forma Card automatically if you enroll in a Forma pretax benefits account, including FSA, LPFSA, DCFSA, HSA, Transit, or Parking account. If you do not receive your Pre-tax Forma Card before your account start date, please reach out to our member experience team.
If you would like to request a card for your dependents, navigate to your Forma portal → Settings → Forma Card and issue a card for your dependents. You can only request a dependent card if you have an FSA, LPFSA, DCFSA, or HSA.
Card Activation
Once you receive your physical Forma Card, please call the number +1-833-333-0417 to activate it and set your PIN. You can reset your PIN anytime after you activate the card by going to Forma card settings page.
Pre-tax Forma Card is Lost, Stolen, or Damaged
Please cancel your Forma Card from your Forma card settings page. Select the "Replace Card" button to cancel and request a replacement card.

Lock the Card
If you do not intend to use the card for a while, you can lock the go on your Forma card settings page. Click on the “Lock Card” button to temporarily lock your card from any activities. You may unlock the card anytime you wish.
What is the Billing Address for my Pre-tax Forma Card?
The billing address is by default the same as your mailing address when it is mailed. You can check and edit your billing address anytime by going to your Forma card settings page and checking the billing info.
The Billing address is usually your primary residential address, which you use when fulfilling online orders. The billing address must match what the card issuer has on file and it allows us to reduce fraud. Please make sure you change it to your current address when you move.
What is the Phone for My Forma Card?
The phone number associated with your Forma Card comes from your profile settings. To ensure you don’t miss any important notifications or text messages regarding your Forma card activities, please make sure to check your phone number is up-to-date by going to Forma portal → Settings → Profile.
Can I make a purchase that costs more than my account balance?
No, you can only spend up to your Forma account balance. To check your account balance in real-time, navigate to your Forma portal → Accounts.
If you only have one account but with insufficient balance, you can ask the merchant to split the payment upon the transaction, or pay out-of-pocket first then submit the reimbursement claims to be partially reimbursed with the remaining balance you have.
