A Pre-tax Forma card is automatically issued when you enroll in your pre-tax spending accounts (HSA, FSA, LPFSA, DCFSA, and Commuter) with your employer. However, if you lose your card and need to order a replacement, you can follow the steps outlined below.
Step 1: Log into your Forma account and go to Settings by clicking your profile photo in the upper-right corner.
Step 2: Go to the Forma Card section, and then click Replace Card.
Step 3: Select the reason for card replacement.
Step 4: Fill out the Shipping Address on the next page.
Step 5: Once you click "Request new card", then you're all set! Forma Cards are usually delivered in about 5-7 business days.

Canceling your current Pre-tax Forma card
If you lost your current Pre-tax Forma card and would like to order a new one, we recommend completing the steps below:
Step 1: Go to Settings by clicking your profile photo in the upper-right corner.
Step 2: Go to the Forma Card section, and then click Lock Card.
Step 3: Once your card has been locked, follow steps 2-5 from the Request a New Card section outlined above.
Tracking your Pre-tax Forma card delivery
Pre-tax Forma cards are mailed by the standard U.S. mail so, unfortunately, there isn't a tracking number associated with your card. We're sorry about that! You can expect to receive it in about 7-10 days.
