Forma provides two methods for accessing your CDH benefit funds: reimbursement claims or a pretax Forma Card. If you choose to pay for eligible expenses out-of-pocket, you can file a reimbursement claim to be reimbursed later.
To file a reimbursement claim, simply sign in to your Forma account on the Forma portal or mobile app, and navigate to “Claims > File new claims”.
You will be prompted to fill out a claim submission form that includes the following fields:

Document: Provide as much detail as possible to show that the expense is eligible under IRS regulations.
After you submit the claim, Forma's dedicated claim adjudication team will review your claim within 48 hours. You will receive a notification of the claim adjudication result via your preferred method. If your claim is rejected, a specific message will be included to explain the reason for the rejection.
If your claim is approved, you will receive reimbursement to your linked personal bank account via direct deposit. Forma processes reimbursements on a semi-weekly basis.
