Following termination of employment, you may have pre-tax expenses that you’d like to be reimbursed for. You can file reimbursement claims post-termination using either of the following methods:
- Reach out to Forma support at firstname.lastname@example.org and provide your first name, last name, and previous work email address and someone from the Support team will provide you with temporary access to your Forma account.
- Reach out to Forma support at email@example.com and provide details about the claim you’d like to file. Include as many of the following details as possible:
- Pre-tax account type
- Merchant or provider
Claim submission is the only accepted reimbursement method during the post-termination run-out period. Your existing Forma Card will be deactivated.
