Welcome to Forma! In this article, we'll show you how to navigate and manage your Forma Benefits Account.
Accounts
Check Account Balance
To check the balance in your account, navigate to the Benefits drop-down located in the main menu at the top of the screen.
Once selected, you can see the list of your CDH Accounts under the Benefits section. When viewing a specific benefit account from the drop-down, you can see the account balance, recent activity, account details and more.
Spend The Money
You have 3 ways to spend the funds in your Forma Benefits Account, listed below.

FSA/HSA Stores
FSA/HSA Stores
To browse around the pre-tax store, click on Store on your Forma main page. When the account is selected, you will see a list of clickable links to a third-party store that provides FSA/HSA eligible items, including Amazon, CVS, Walgreens, FSA Store, and HSA Store.
Note: These stores are only available for users who have an FSA, HSA only, or an HSA combined with LPFSA.
After clicking on the selected store, you will be directed to an intermediate page. This page opens on a new tab that informs you that you are leaving Forma and entering the selected FSA/HSA store.

If continue is clicked, the selected FSA/HSA store is opened on a new tab.

If you don't want to continue but stay on the same Forma page, simply close the tab.
Note: These stores are our third-party vendors. Once directed to the specific store, you would handle transactions with them directly, so you would need to enter your benefits card or personal payment information upon check-out. All the orders will be fulfilled by the vendors.
New Reimbursements Claim
If you choose to pay out of pocket for an eligible expense, make sure to retain the receipt and submit a reimbursement claim, which will be processed within 2-3 days within the Forma portal.
Simply file a manual claim to get reimbursement by going to Forma Portal > Claims > File new claim.
Fill in the information required on the following page.

Select the account you are filing the claim for.

Claim Amount.

Enter Merchant or Provider Name.

Select Category.

Enter Claimant.
You can file a claim for yourself or on behalf of your dependent. If filing for your dependent, simply add the name right here.
Read this Article to see who can be covered by your benefits accounts. -
Enter Service Start Date or Items Purchase Date. The date must be on or after your Plan Effective Date, or Coverage Start Date. Check the date in your ben-admin system.

Upload your receipt. (Max. upload size: 10MB per document, up to 30MB in total)

Add a note for your purchased service/item.

Note: If you need to add an HSA expense, please do not submit via claims, as Forma will not adjudicate your HSA claim. See here: How to manage your HSA expenses?
Benefits Card
See Benefits Card FAQ for more details.
Card Information
Find out your Benefits Card information by going to Setting > Your Cards > Benefits Card. You can request a new card and cancel an existing card on this page, or find out the last four digits of your Benefits Card. As Benefits Card is a physical card, you can always find the card numbers, EXP. Date, and CVC when you receive the card.
You can also find out what benefits accounts are associated with your Benefits Card. Funds of all your benefits accounts are loaded on the same, single Forma Benefits Card.
Dependent Card
Once you create a dependent by either filing a claim or requesting a card, you will see the dependent under your profile Settings > Dependents. You can edit the information for your dependent.
