This only applies if you have a Forma HSA in 2021/2022 with WealthCare Saver.

if you've invested your Forma HSA money, you'll need to liquidate those investments and have the money back in your HSA spending account before making the transfer. You can refer to this article for instructions
Steps:

Reach out to your new custodian bank for their transfer form.

Their form will likely ask for your Forma HSA account number. If you don't know the number, you can check online through your Forma account.

Log into Forma. Go to Forma → Accounts → Health Savings Account → Manage HSA → Pay Myself. This will redirect you to our partner site where you'll see the last 4 digits of the account number at the top of the page. Click the eye icon to reveal the whole number.

If the form asks for Forma's custodian bank information, provide the following:
