Virtual Forma Card Expiration
Before your Virtual Forma Card expires, Forma will automatically renew it. When the new card is activated, the old one will automatically be deactivated for you. No action required!
Physical Forma Card is Lost or Stolen
You can replace or cancel your physical card directly from your Forma account!
On the web
- Log in to your Forma account
- Click on your name in the top right corner, and select “Manage Forma Card”
- Select “Report lost or stolen” and you can choose to cancel the card or get a replacement sent to your mailing address.
On the app
- Log in to the Forma app
- Tap “Settings”, then “Forma Card”. Swipe to the right to see your physical card, then tap “Replace” and you can choose to cancel the card and get a replacement sent to your mailing address.
