Forma Card
The Forma Card is a prepaid debit card linked to your eligible Forma spending account(s). You may use the card anywhere VISA is accepted to purchase eligible items and services. Please first consult your company's program policy to see if you are eligible for the Forma card.
***Your Forma Card is linked to your Forma account and the transactions are subject to the approval rules set by your company. To see a list of eligible and ineligible card transactions, please consult your company's program policy. Transactions may be rejected if they are considered ineligible under the policy.
Types of Cards
We offer 2 types of cards: virtual and physical. Depending on the options your company chooses to enable, you may be able to request one or both types of cards.
Virtual Cards
If you request a virtual card, the card is immediately available for use. You can view your card information directly in your Forma account by clicking on Settings from the dropdown menu in the upper right hand corner of your homepage, and selecting Forma card.
The virtual card can be used online or in-store by adding the card to your mobile wallet. See here for instructions on how to add your virtual card to your mobile wallet.
Physical Cards
If you request a physical card, the card will be shipped to the address you provided on the card request form in 7-10 business days. The card is immediately available for use -- no need to activate it!
***Please note note that your card number will not be available in your Forma account due to security reasons.
How Can I Apply For a Forma Card?
If your company offers the Forma card, you can request one through your Forma account.
Simply navigate to your Forma account and click on Settings from the drop down menu located in the upper right-hand corner. If your company has enabled the Forma Card option, you will find the Forma Card tab located on the left menu panel.
Once you have arrived on the Forma Card page, simply click on the Add Card button to request a card. Once the card is issued, you will also find all the relevant settings under the Card Settings tab.
Once you have added your card, you will be able to see which wallets are linked to your Forma account below.
Is the Forma Card a Debit or Credit Card?
It's a debit card! It works just like any normal debit card; however, it can only be used on products and services that are eligible under the policy set forth by your employer and only up to the balance available in your Forma Wallet.
Does My Card Have a PIN Number?
No. There is no pin number associated with your Forma card. If you run into an issue while using your card, try to run the card as credit.
Where Can I Find My Forma Card Balance?
Your Forma Card balance is connected to your Forma Wallet(s). Please navigate to your Forma account and click on the Accounts tab in the top navigation bar to locate your balance.
Where Can I Use My Forma Card?
We recommend adding your virtual card or physical card to your mobile wallet on iOS and Android. This allows you to pay in the physical world where smart payments are accepted. Where you can use your Forma Card is determined by your employer's program policy. We recommend referencing your program policy to ensure eligibility.

Can I make a purchase that costs more than my wallet balance?
If you would like to purchase an item with your card that costs more than your wallet balance, you can add a personal credit card to your Forma account as a backup payment method. To learn how to add your personal payment information to your account, please see the instructions, here.
Then, make sure that the "overdraft" feature is enabled for your Forma card. To find this, navigate to the Forma card page in your account and select Card Settings. Then ensure that the overdraft toggle is on.
